EFFECTIVE July 1, 2009

One thing which is very important to my own Self-Health, is knowing I can provide the same quality of assistance to those who are experiencing financial hardship as I offer to those who are financially capable of affording my services.    At this time, I am able to provide $10,000 worth of Grant Supported Services.  The distribution of Grant Funding is as follows:

• $1,900 in The Elevator Pass Co-op Services.  The fund will provide the 3 month program to 12 Clients.  (Co-Pay=$31.60/client for 3 month Program)

• $2,100 in Hot Line Services.  The fund will provide 4 months of Hot Line Benefits to 7 Client.  (Co-Pay=$60/client for 4 months of services)

• $3,600 in 1--2-1 On Call Pay-by-Day Facilitation Services  The fund will provide 3 months of Facilitation Services to 4 Clients.  (Co-Pay=$180/client for 3 month Program)

• $2,400 in Self-Health Assurance coverage.   The fund will provide 1 year of Self-Health Plan In Network Services to 1 Client. (Co-Pay=$480 for 1 Year Program)

Clients agree to an out of pocket co-payment of 20% of of the funded amount.  Two installment payments can be arranged.
